Community Mental Health Social Worker – Band 6

Hertfordshire Partnership University NHS Foundation Trust is seeking a qualified Band 6 Social Worker to join our community mental health team serving the Dacorum area (Hemel Hempstead, Berkhamsted, Tring and surrounding villages).

You will carry out assessments, care planning and coordination for adults with mental ill‑health, champion equality and safeguarding, and work within a multi‑disciplinary team to promote recovery, independence and social inclusion.
