Flexible Lead District Nurse – Community Health & Case Management

Buckinghamshire Healthcare NHS Trust is seeking a Band 6 nurse to work within the Marlow Adult Community Health Care Team (ACHT). The role combines district nursing with community case management to assess physical and psychosocial needs and prevent hospital admissions.

You will work under a community nursing matron and district nursing team lead, managing a caseload of patients with long-term conditions and supporting timely discharge and end-of-life care in the community.
