Primary Care Mental Health Nurse — Integrated Care Catalyst

East London NHS Foundation Trust is recruiting a Staff Nurse to join a Primary Care Mental Health Practitioner team in inner London. The post supports GP practices and works within a blended team to deliver integrated mental and physical health care.

You will assess, plan and deliver patient care with professional accountability, contributing to transformation of community mental health services across the PCN footprint. Experience in mental health settings is desirable.
