Job Description:
National Health Service in Darlington seeks a Clinical Pharmacist to support Enhanced Health in Care Homes. You will work with Pharmacy Technicians delivering Structured Medication Reviews, medicines optimisation across care homes and GP practices, and advise on covert plans and queries.
You may be an independent prescriber or complete training; focus on polypharmacy, frailty, COPD/asthma, and learning disabilities. You’ll lead on antibiotic stewardship and quality improvement within the PCN.
