Social Worker – Independence & Recovery Hub (Trusted Assessor)

London Borough of Enfield is looking for an experienced Social Worker to join the Assessment and Prevention Hub in the Older People and Physical Disabilities Service. You will promote independence, support recovery, and improve outcomes for residents in partnership with health colleagues.

You will carry out strengths-based assessments, develop person-centred support plans, and work with health professionals, families, and community partners to deliver timely, effective care.
