Community Mental Health Social Worker – Hybrid (12-Month FTC)

East Midlands Councils/East Midlands Jobs is seeking a Social Worker for the Erewash Mental Health Social Work Team. The role involves assessing adults 16+ in the community, applying Care Act 2014 and Mental Capacity Act 2005 to develop strengths-based care plans.

You will work with service users and their networks to promote independence, applying person-centred approaches and keeping safeguarding at the core. Hybrid working with office/home balance and ward visits as required.
