Job Summary:
The Hospice Community Nurse (HCN) will work as part of the community nursing team, delivering a specialist advisory service through the management of a caseload within the community setting.
The HCN will report to the Band 7 nurse for day-to-day operational oversight, support and guidance.
The HCN will act as a specialist resource for healthcare professionals in primary care, the Northern Ireland Hospice multidisciplinary team, Trust multidisciplinary teams, other agencies and individuals involved in patients’ care.
