Primary Care NP/PA – Patient-Centered, Team-Based Care

Rosaires Care seeks a Nurse Practitioner or Physician Assistant to provide patient-centered primary care within a physician-led team. You will conduct assessments, develop care plans, diagnose and manage conditions, and coordinate across services.

You will prescribe medications, order tests as needed, and deliver preventive care while supporting families and individuals in the community. Collaboration with physicians and behavioral health teams is essential to integrated care.
