Senior Community Mental Health Practitioner – Recovery-Focused Care

NHS Jobs is seeking a Senior Community Mental Health Practitioner to deliver high-quality mental health services. The post holder will manage a defined caseload, providing expert assessment and treatment for individuals with complex mental health needs and their carers.

You will coordinate recovery plans and care packages, with personalisation and evaluation in line with payment by results, and provide statutory Adult Social Care functions such as assessment, care planning and reviews.
