Primary Care Mental Health Liaison Specialist

East London NHS Foundation Trust is seeking a Primary Care Mental Health Link Worker to join its CMHT-based team. The postholder will work with GP practices to provide assessment, advice, and problem-solving support for adults with complex mental health problems in primary care.

You will help develop integrated care models, promote personalised care, and facilitate referrals to specialist and voluntary services, while engaging in supervision and MDT collaboration with local services.
