Remote Social Worker – Hospital Discharge & Care Transfer

The Derbyshire County Council invites applications for a Social Worker within the Out of Area Hospital Discharge Team (OOA HD). You will assess Derbyshire residents in acute hospital settings and plan discharges under the Discharge to Assess model, coordinating informal services, homecare and therapy beds.

Working across countywide services, you will join a multidisciplinary team, balance autonomous practice with team collaboration, and adapt to evolving care pathways.
