Remote Community Case Manager – Mental Health Support

Center for Life Management is seeking an Adult Community Case Manager to provide care planning, resource coordination, and community-based support for adults 18+. The role emphasizes collaborative care with internal programs and external partners, and includes home-based counseling, crisis intervention, and documentation in line with agency standards.
