Lead Community Assertive Outreach Team (Band 7)

CNWL NHS Foundation Trust in London is seeking a Band 7 Team Lead to join our Assertive Outreach Team (AOT) in Hillingdon. You will lead a multidisciplinary team delivering proactive, home-based care to individuals with severe mental illness, ensuring no one is lost to follow-up.

The role emphasises patient engagement, leadership, and high-quality risk management in a community setting. Remote working is not suitable for this position.
