Regional Clinical Care Leader – Complex Home Health

Active Care Group is seeking an inspired Clinical Manager to lead complex home-based care packages across a region. You will shape clinical care plans, guide the team including care managers and coordinators, and ensure safe, high-quality outcomes in people’s own homes.

You will bring RN with NMC registration, plus governance and risk-management experience, and you will participate in MDTs, funder meetings, and on-call rotations as required.
