South Lanarkshire Council is seeking a permanent Social Worker to join the Hospital Discharge Social Work Team at University Hospital Wishaw. The role focuses on adults and older people needing support to discharge from hospital, with responsibilities spanning assessment, risk management and care-planning within acute and community settings.
You will work within a MDT, liaising with families and carers, and carrying out safeguarding duties under Adults legislation.
