We’re looking for a Social Worker to join our team. We are looking to appoint as a Level 2, Level 3 Social Worker or Senior Practitioner.

About SAPT
The Specialist Adult Pathway Team (SAPT) supports young people and their families as they prepare for adulthood. We work closely with Children with Disabilities (CWD) and wider Children’s Social Care teams, alongside health, education and other partners, to make sure the transition into adult services is as smooth and well planned as possible.
You’ll be joining a team where you can build meaningful relationships with young people and families and make a real difference at an important point in their lives.
