Job Description Are you an experienced Care Coordinator looking for a role where you can take ownership of your rotas, support your care teams and play a key part in delivering high-quality care within the community?n n We are looking for a Care Coordinator to join an established homecare provider in Bournemouth. You’ll work Monday to Friday, 9am–5pm, alongside additional on-call duties on a rota basis.n n As a Care Coordinator , you’ll be responsible for:n n n Coordinating care rotas and ensuring all visits are effectively covered n Supporting and supervising care workers in the community n Assessing and reviewing individual care plans n Completing field-based visits with clients and care staff n Building strong relationships with clients, families and care workers n Responding to changes in client needs and resolving day-to-day issues n Ensuring care is delivered safely and to a high standard n You’ll need previous experience within care coordination or a similar community care role, with strong organisational and communication skills and the ability to manage busy rotas effectively.n n A full UK driving licence and access to your own vehicle are essential.n n Alongside a salary of up to £28,000 , you’ll receive 22 days holiday plus bank holidays, ongoing training and qualifications, retailer discounts, occupational sick pay and opportunities to progress within a large healthcare group.n Apply today to be considered for this Care Coordinator position, or call Callum on 07445 309375 for more information. n n JBRP1_UKTJ
