Primary Care Pharmacist – Medicines Optimisation Lead

Islington GP Federation in London is seeking a pharmacist with experience in primary care to develop and manage Medicines Optimisation services within general practice. The post-holder will work in a multidisciplinary team providing clinical medicines advice and leading on safe prescribing and polypharmacy reviews.

You will coordinate with community and hospital pharmacy, support repeat prescribing systems, and contribute to quality improvement and patient safety.
