Transitions Social Worker (14-25) – Hybrid Role

Bristol City Council is hiring a Social Worker for the Young Adults Transitions Service in Bristol. This fixed-term role (12 months), 37 hours per week, offers a multidisciplinary environment supporting young people aged 14-25 as they transition to adulthood.

You will conduct Care Act assessments, support conversations and plans, and work with partners across Adult Social Care to secure non-commissioned support and services. Hybrid working is available with office presence at City Hall.
