Autonomous Community Nurse – Home Visits and Care

Lincolnshire Community Heath Service (LCHS) is looking for a Registered Nurse to join the Lincoln North Community Nursing Team. The role involves autonomous practice and teamwork to deliver nursing care in patients’ homes, care homes and clinic settings.

You will assess care needs, plan and evaluate care using evidence-based practice and work closely with the MDT to coordinate patient care and improve outcomes.
