CCH Group in Belfast is seeking a Care Coordinator to organize and deliver high-quality home care across the community. The role involves coordinating referrals, creating rotas, and liaising with social workers and GPs to monitor service users’ conditions.
We value organized individuals with healthcare awareness and knowledge of CQC, care plans and rotas. Monday–Friday 9am–5pm with on-call duties on a rota. Salary £25,967.
