Community Mental Health Clinician – Home Visits

The Community Mental Health Service (CMHS) in Plymouth, part of Livewell Southwest, is seeking a post-holder to assess needs, develop and evaluate care, and deliver holistic biopsychosocial support.

You will supervise junior staff, coordinate with inpatient and social care teams, and produce risk formulations and care plans in partnership with patients and carers.

Care is delivered in patients’ homes and community settings, with flexible 7-day patterns including on-call nights if required.
