Job Description:
The Central and North West London NHS Foundation Trust is seeking a Band 5 District Nurse to join the DN team in Hillingdon for an 8-month fixed-term period. You will visit patients in their homes, assess needs and deliver care with a focus on community-based outcomes.
You must be an NMC registered nurse, enthusiastic about autonomous working in the community, and prepared to work shifts including weekends. A literacy and numeracy assessment will be part of the shortlisting process.
