Hybrid Adults Social Worker: Discharge & Reablement

Sanctuary Personnel are seeking an Adults Social Worker in Croydon to support adults post-hospital discharge within a hospital discharge team. The role offers hybrid working with both full-time and part-time options, paying £36 per hour.

You will assess, plan, and coordinate with healthcare professionals, families, and carers to ensure safe discharges, with responsibilities including Reablement, Discharge to Assess, and Care Act compliance. Experience with Liquidlogic LAS is beneficial.
