Senior Social Worker – Key Responsibilities

• Conducting thorough adult assessments to determine individual needs
• Developing and implementing relevant care plans to support client independence
• Collaborating with multidisciplinary teams to coordinate care services
• Providing expert advice and guidance to junior team members
• Ensuring compliance with safeguarding policies and legal frameworks
• Maintaining accurate and detailed case records
• Participating in team meetings and contributing to service improvement initiatives

Candidate Requirements

• Proven experience in adult assessment and care management
• Strong understanding of social work principles and legislation
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Ability to manage a demanding caseload efficiently
• Registration with the HCPC (Health and Care Professions Council)
