Youth Housing Support Lead: Case Planning & Care

mbi Medical Ltd is seeking an experienced lead for case and care planning management for children and young people. You will assess needs and risks, develop care plans, and carry out regular reviews in collaboration with young people and professionals.

On shift, you will take accountability for service delivery, manage staff and resources, supervise and mentor colleagues, and ensure safe care in line with policies and regulatory requirements.
