Senior Band 6 Home Care Nurse (Acute Community)

Sirona care & health CIC is seeking an experienced district/community nurse to join NHS@Home across Bristol, North Somerset and South Gloucestershire. You will triage patients, coordinate care tasks, and monitor patients in their homes using a range of clinical skills and digital tools.

You’ll lead care plans with the MDT, assess needs across hospital and community settings, and deliver acute care including IV therapy and wound management. NHS pension and benefits apply.
