Care Coordinator: Scheduling & Client Care

Team Medical in Southampton is recruiting a Care Coordinator to join our friendly team based at our Southampton Offices in Redbridge. Office hours are Monday to Friday 08:30 – 17:30, finishing at 17:00 on Fridays.

You will plan schedules a month ahead, match staff skills to client needs, and liaise with external organisations and families while maintaining capacity plans and providing written reports on incidents.
