Locum Adult Social Worker – Discharge and Care Coordination

Sanctuary Personnel is seeking a dynamic Social Worker to facilitate safe hospital discharges and manage the Extra Care Housing Step-Down Pathway in Bromley. The role emphasises strengths-based practice, timely reviews, and coordinated discharge planning with health and social care partners.

You will assess residents under Care Act 2014 and Mental Capacity Act 2005, balance risk and independence, and work across multidisciplinary teams to deliver person-centred support.
