Care Navigator: Proactive, Coordinated Patient Support

Windsor Primary Care Network (PCN) is seeking a motivated and compassionate Care Coordinator to join our multidisciplinary team, delivering proactive, personalised care to our local community and helping patients navigate health and care services.

You will work with GP practices and wider teams to identify those who may benefit from additional support, coordinate care across organisations, and promote independence and wellbeing.
