Senior Dementia & Geriatric Community Nurse (Travel)

Cornwall Partnership NHS Foundation Trust seeks a qualified RMN to join our Dementia & Older People’s Mental Health Service. The post supports adults with dementia and functional mental health needs, including travel across Cornwall to visit patients at home, in care homes and clinics.

The role requires a senior CPN to work with a multi-disciplinary team, manage caseloads under the Care Programme Approach, and collaborate with carers and services in the community to provide person-centered care.
